What Animal Hospital actually is

The premise puts you on the night shift at a veterinary hospital for humanoid animals, working alongside up to three other players. On the surface it feels like a calm experience centered on caring for patients: you check them in, examine them, and give them the right treatment. The twist is that some of those patients aren't what they appear to be. Certain visitors are actually anomalies disguised as normal patients, creatures roaming the hospital that can attack both real patients and other players.

The job runs against the clock: you admit patients while making sure they aren't an anomaly, walk them to their assigned room, and complete their treatment within a time limit, all while running down the hallway for supplies whenever you need them.

The three detection methods

The most important decision in the game happens before a patient ever enters the building. Animal Hospital gives you three ways to check them, and none of them catches everything on its own: the patient's appearance through the window, the printed photo on their admission file, and the security camera (CCTV) feed. Each method catches threats the other two miss, so the only reliable way to play is running all three checks on every single patient, no exceptions, before deciding whether to admit them.

Visual signs that a patient is an anomaly

There are several concrete signs worth learning to recognize: a glowing orange or red third eye sitting above the normal two, a hollow face with completely empty eye sockets, an unnaturally wide grin paired with bulging eyes and heavy eyebags, or realistic human teeth instead of a typical animal mouth. There are audio cues too: a distorted voice, or one pitched several octaves deeper than that animal should sound, is a clear red flag that something's off.

None of these signs show up obviously every time, which is why calmly checking the window, the photo, and the camera before admitting each patient matters more than speed. Players who rush admissions without running all three checks are the ones who lose the fastest.

Some of the most dangerous anomalies

Not every anomaly carries the same level of risk. The Skinwalker is considered the most dangerous CCTV anomaly: a black figure with glowing red eyes and sharp teeth that shows up in the security booth feed, which usually zooms in on it automatically, so switching the camera off fast is key to avoid losing sanity all at once. The Bed Monster is another well-known threat, recognizable by the long black hands that reach out from underneath hospital beds; staying inside the red danger zone around the bed lets it grab you and drag you while draining your sanity. The Stalker, the ceiling entity, and the wall-based anomalies are more routine threats, but still perfectly capable of chipping away at your sanity if you ignore them.

Letting an anomaly in without noticing doesn't stop there — it can trigger death rituals, spawn additional monsters, or set off a direct attack from the Mimic. And if your sanity hits zero for any of these reasons, you die instantly, no matter what point in the shift you're at.

The available classes

You start the game with the free Intern class, which lets you play with no restrictions but no special perks either. As you play and earn Animal Coins, the game's currency, you can buy additional classes like Nurse, Secretary, Surgeon, and Doctor, each with its own focus within the hospital. If you're struggling to survive shifts, defense- or speed-oriented classes like Security or Nurse tend to make things noticeably easier while you're still learning to spot anomalies consistently.

How you lose a shift

There are two ways to lose a shift in Animal Hospital: your sanity hits zero, or three patients die during the same shift. Both are usually a direct consequence of admitting anomalies without checking them properly, so discipline in your inspection process is literally what separates players who survive from those who don't.
